Welcome to the Kutztown Area School District. Below is a list of documents which must be reviewed, completed, signed, and returned to the Human Resources Office of the Administration Building as soon as possible following board approval. 

• Act 24 Arrest/Conviction Report and Certification Form
• School Employee/Volunteer Disclosure Statement
• School Personnel Health Record Form (within one year) with TB Test Results (within three months)
• Federally Mandated Drug Free Workplace Statement
• CONNECT ED Notification Form (optional)
• KASD Employee Health Update Form
• Policy 815 Acknowledement and Consent Form (after review of Policy 815)
• Mandatory SafeSchools Online Training Modules
